Create Notification Form

The notification form is to be used for projects that are subject to notification under the Personal Data Act. Enter your e-mail address to create a notification form.

E-mail:

You will receive an e-mail with a link to your notification form. Completion can be made in several stages. The notification form must be submitted no later than 30 days prior to the processing of personal identifiable data. If you decide to make changes to the project after is is processed, you must submit a separate change request form.

Obligation to Notify?

Personal Data?

If you will be collecting/recording/storing personal data by means of a computer, your project is subject to notification. Any information that may be linked to a person is considered personal data. The information may for instance be linked to a person through name or through a number referring to a list of names, or through a combination of background information.

Sensitive Personal Data?

If your data material contains sensitive personal data, your project may be subject to notification even if the information is not processed on a computer. It is sufficient that the personal data is stored systematically, so that information concerning an individual may be retrieved.

Audio/Video Recordings or Photographs?

Will you be using a computer to process/store video recordings or photographs of individuals? Or, will you process/store audio recordings, where individuals can be identified directly or indirectly? If so, your project is subject to notification.

Online Surveys?

If you will be collecting data through online surveys, you have an obligation to give notification if the data material is connected to an IP address, e-mail address and/or directly or indirectly identifiable personal data. Note that you have an obligation to give notification even though you (as researcher or student) do not gain access to the alignment of data to the IP- or e-mail address, as is often the procedure when using a data processor.
